Description
Opened within the right atrioventricular orifice, the tricuspid valve leaflets (anterior, posterior, and septal) separate to form a broad, triangular inlet from the right atrium into the right ventricle. Chordae tendineae course inferiorly from the free margins of the cusps to the anterior, posterior, and septal papillary muscles, which project from the ventricular trabeculae carneae. Medially, the septal leaflet lies against the interventricular septum, while laterally the annulus follows the right ventricular free wall. Thin atrial endocardium transitions into thicker ventricular myocardium at the valve ring. This open-valve state corresponds to ventricular diastole, when pressure in the right atrium exceeds the right ventricle and passive filling predominates. It is the surgical and echocardiographic anatomy that matters in functional tricuspid regurgitation, where annular dilation and right ventricular remodeling prevent leaflet coaptation, and in endocarditis where vegetations often form along the atrial aspect of the leaflets and chordal insertions. The septal leaflet’s relationship to the membranous septum and conduction tissue also frames why septal annular sutures demand restraint during tricuspid repair.
