Description
Lateral cranial anatomy is rendered to emphasize the sigmoid sinus as it curves in an S-shaped groove along the posterior aspect of the petrous temporal bone and mastoid part of the temporal bone, continuing inferiorly as the internal jugular vein at the jugular foramen. Superiorly and posteriorly, the channel relates to the transverse sinus as it leaves the tentorial attachment on the occipital bone and turns anteroinferiorly toward the mastoid region. The adjacent squamous temporal bone and greater wing of the sphenoid frame the lateral cranial wall, while the dural venous sinus itself sits between periosteal and meningeal layers of dura mater, immediately deep to the inner table. A tight corridor. Clinical relevance concentrates in the mastoid segment. Otitis media and mastoiditis can propagate via emissary veins to cause sigmoid sinus thrombosis, and the close relationship to the mastoid air cells explains why headache, fever, papilledema, and sixth nerve palsy may accompany an otogenic intracranial complication. Surgeons also track the sigmoid sinus during mastoidectomy, translabyrinthine approaches, and posterior fossa exposure, where its anterior border and the sinodural angle serve as practical landmarks to avoid venous injury and brisk hemorrhage.
