Description
Visible from an inferior aspect, the cerebral venous system is rendered transparently over the ventral surface of the male brain, with superficial cortical veins converging toward the dural venous sinuses and deep tributaries tracking medially toward the internal cerebral veins. Blue venous channels course posteriorly to form the great cerebral vein (of Galen) beneath the splenium region, then drain toward the straight sinus at the tentorial junction. Basal veins (of Rosenthal) run along the medial temporal and midbrain contours, linking deep drainage from the insula, diencephalon, and mesencephalon to the Galenic system. Relationships read clearly, deep veins remain central and medial, while larger superficial collectors sweep laterally across the inferior temporal and frontal surfaces. Deep venous anatomy matters when you need to localize a hemorrhage or venous infarct to a drainage territory rather than an arterial distribution. Thrombosis of the internal cerebral veins or the vein of Galen can produce bilateral thalamic edema and decreased level of consciousness, a pattern that is easy to miss if you only think in terms of the circle of Willis and major arteries. Surgical corridors to the pineal region and posterior third ventricle also live and die by this map, because injury or occlusion of the internal cerebral veins, basal veins, or Galenic confluence can be catastrophic. Small targets, big consequences. Neuroanatomy and neuroradiology teaching benefit from this inferior perspective when correlating MR venography, CT venography, and catheter angiography, since it reinforces how superficial and deep cerebral circulation interconnect rather than behaving as separate systems. Publishers can drop the artwork into chapters on cerebral circulation, dural venous sinus thrombosis, pineal region surgery, or venous drainage of the thalami and basal ganglia.
