Description
Posteriorly, the hepatopancreatic ampulla (ampulla of Vater) is presented where the distal common bile duct descends to meet the main pancreatic duct (duct of Wirsung), forming a short common channel within the wall of the second part of the duodenum. Superior and slightly medial to the ampulla, the supraduodenal and retroduodenal segments of the bile duct course toward the pancreatic head, while the pancreatic duct runs from the body and tail toward the posteromedial aspect of the gland. At the duodenal entry, the major duodenal papilla and the surrounding sphincter of Oddi complex are implied landmarks. Ductal confluence is the focal point. A posterior perspective matters because it mirrors how the distal bile duct is encountered in relation to the pancreatic head, portal triad structures, and the retroperitoneal plane during operative and endoscopic planning. Obstruction at the ampulla by an impacted gallstone can produce gallstone pancreatitis, and small ampullary adenomas or carcinomas can cause painless obstructive jaundice with upstream dilatation of both ducts (the double-duct sign on MRCP or ERCP). Clear orientation of the common channel also helps explain why sphincterotomy at the major papilla can decompress both the biliary tree and the pancreatic duct, yet carries a recognized risk of post-ERCP pancreatitis.
