Description
Arising within the submucosal venous plexus of the lower lip, the inferior labial vein courses laterally and slightly inferiorly from the midline toward the oral commissure, then turns posteriorly to join the facial vein near the modiolus. Superficial to the orbicularis oris but deep to the lip skin, it runs in close company with the inferior labial artery and small venules that communicate across the vermilion. Adjacent planes include the depressor anguli oris and depressor labii inferioris inferiorly, with the mental foramen and mental nerve lying posterolateral to the vein’s expected trajectory. Small, variable tributaries may pass superiorly toward the superior labial system or inferiorly toward the submental region. Anterior exposure of the inferior labial vein matters because venous anatomy at the lower lip is both variable and clinically unforgiving when you cut across it. Brisk bleeding during lower-lip laceration repair, wedge excision, or transoral approaches can come from this vessel or its plexiform tributaries, and venous caliber can be exaggerated in venous malformations or post-traumatic varicosities. The relationship to the facial vein also anchors a teaching point: infection and thrombophlebitis in the perioral region can track along facial venous channels, and appreciating drainage patterns helps explain how facial infections can spread beyond the lip. A small vessel, but not a trivial one. Plastic and reconstructive surgery texts can pair this plate with chapters on cheiloplasty, vermilion border repair, and local flap design to emphasize safe planes around the modiolus and preservation of neurovascular supply.
