Description
Positioned in the lower anterior face just deep to the dermis, the depressor labii inferioris (historically termed quadratus menti) lies lateral to the mentalis on the chin and inferior lip. Its fibers arise from the oblique line of the mandible and ascend superomedially to blend with orbicularis oris and insert into the skin and mucosa of the lower lip, sitting inferior to the oral commissure and medial to depressor anguli oris. An anterior perspective keeps the right and left muscle bellies symmetrical around the midline, with the mentalis forming a more central, paired prominence over the mental protuberance. Superficial landmarks remain readable. Beard and facial hair help orient the lower third of the male face. Depressor labii inferioris matters whenever you need to explain asymmetric lower lip motion, drooling, or altered articulation after facial nerve injury. Weakness of the marginal mandibular branch can denervate this muscle, leaving the lower lip pulled upward and laterally by intact elevators, a pattern clinicians often test during a forced smile, pouting, or showing the lower teeth. In aesthetic practice, botulinum toxin to the depressor labii inferioris is used to reduce a “gummy” lower dentition show or to balance a hyperactive depressor complex, but over-treatment can blunt speech and oral competence. Small muscle, big functional consequence.
