Description
Positioned deep to the skin of the lower face, the depressor labii inferioris (quadratus labii inferioris) fans superiorly from the oblique line of the mandible toward the lower lip, where its fibers blend with orbicularis oris near the labial commissure. In lateral profile, it sits anterior to the masseter and inferior to the buccinator, with the mentalis occupying a more medial, chin-centered position. The mandibular body and dentition form the bony scaffold immediately deep to the muscle, while the hyoid and upper cervical vertebrae lie inferior and posterior in the same plane. Functionally, this muscle pulls the lower lip inferolaterally, opposing elevators such as levator labii superioris and coordinating with depressor anguli oris to shape expressions of sadness, doubt, or tension. Small changes matter. Weakness from marginal mandibular branch of the facial nerve injury after submandibular gland surgery, rhytidectomy, or mandibular fracture fixation produces a characteristic lower lip asymmetry that becomes obvious during speech and smiling. The lateral view also helps distinguish depressor labii inferioris from platysma and depressor anguli oris, a frequent source of mislabeling in teaching sets and a practical issue when planning botulinum toxin injections for perioral dystonia or post-stroke synkinesis.
