Description
Shown from a lateral male head and neck perspective, the depressor labii inferioris is isolated deep to the skin and superficial fascia, lying inferior to the vermilion border of the lower lip and anterior to the mandibular body. Its fibers course superomedially from the oblique line of the mandible toward the lower lip, blending near the orbicularis oris at the oral commissure. Adjacent muscles of facial expression remain visible for orientation, including the depressor anguli oris posterolaterally and the mentalis more anterior and inferior on the chin. Bony context comes from the mandible, with the cheek and perioral soft tissues providing surface landmarks. This muscle matters whenever lower lip position is being assessed, because depressor labii inferioris acts to pull the lower lip inferiorly and laterally, counterbalancing elevators such as levator labii superioris and zygomaticus. Weakness from marginal mandibular branch (facial nerve, CN VII) injury, a known risk in submandibular incision approaches and parotid surgery, produces lower lip asymmetry that can be mistaken for central facial palsy. Small anatomy, big clinical consequence.
