Its fibers arise from the oblique line of the mandible and pass superiorly to blend with the orbicularis oris and the soft tissues of the lower lip, sitting superficial to the depressor anguli oris but deep to the dermis and subcutaneous tissue. Superior to the muscle you can infer the course of the modiolus region at the oral commissure, while posteriorly the mandibular body and adjacent facial expression muscles provide orientation along the jawline. This is expression anatomy, not masticatory muscle. Depressor labii inferioris matters because small changes in its tone or innervation produce obvious lower lip asymmetry. Injury to the marginal mandibular branch of the facial nerve during submandibular gland surgery, facelift, or mandibular angle approaches can denervate this muscle, leaving an ipsilateral lower lip that fails to depress during smiling or showing teeth. The muscle also anchors many lower lip and chin injections, since toxin spread into adjacent depressor anguli oris can exaggerate oral commissure droop. Subtle, but clinically unforgiving.
