Presented from the anterior aspect of a male hand, the palmar surfaces of the metacarpals and phalanges frame the interossei muscles occupying the intermetacarpal spaces. Dorsal interossei lie deeper between adjacent metacarpals, while the palmar interossei sit closer to the metacarpal shafts, their fibers converging toward the bases of the proximal phalanges and the extensor expansions on the radial or ulnar sides depending on the digit. Superiorly, the distal carpal row anchors the proximal ends of the metacarpals, and inferiorly the interossei relate closely to the metacarpophalangeal joints where their tendinous insertions blend with periosteum and the extensor hood. Cartilage is indicated at the articular margins. Small spaces, tight mechanics. Functionally, the interossei are the principal controllers of finger abduction and adduction at the metacarpophalangeal joints, and they also contribute to metacarpophalangeal flexion with interphalangeal extension via the extensor mechanism. This anterior presentation is the clearest way to teach the clinical meaning of “interosseous wasting” seen in ulnar neuropathy, where loss of interossei leads to impaired finger adduction, a positive Froment sign, and progressive clawing when combined with lumbrical weakness. Surgeons also reference these planes during dorsal and volar approaches to the metacarpals, where safe dissection respects the interossei origins along the metacarpal shafts and avoids destabilizing the metacarpophalangeal joint capsule.
