Description
Running vertically between adjacent transverse processes, the intertransverse ligaments span the intertransverse spaces of the lower thoracic and lumbar vertebrae and continue inferiorly toward the lumbosacral junction. From a posterior perspective, the paired ligament bands sit lateral to the spinous processes and laminae, forming a thin, segmental connection between transverse processes that are widest and most robust in the lumbar region. Inferiorly, the sacrum and posterior iliac crests frame the base of the vertebral column, with the sacroiliac region positioned posterolateral to the sacral ala. Thickness and fiber direction of the intertransverse ligaments vary by spinal region, a point that matters when you teach coupled motion and when you interpret posterior element pain generators in the setting of lumbar instability. Their intimate relationship to the intertransversarii and the quadratus lumborum means they are encountered indirectly during posterior and posterolateral approaches, and they sit near the corridor used for far-lateral lumbar discectomy where the exiting nerve root and dorsal ramus branches demand respect. Small structures, real consequences.
