Description
Running along the lateral margins of the lumbar vertebrae, the intertransverse ligaments appear as thin fibrous bands spanning between adjacent transverse processes from roughly L1 through L5. From an anterior perspective, the vertebral bodies and intervertebral discs sit medial to these bands, while the iliac fossae and iliac crests frame the lower lumbar region laterally. Inferiorly, the ligamentous attachments approach the lumbosacral junction, where the sacrum nests between the right and left hip bones, with the sacroiliac region forming the pelvic base behind the acetabula. Fine and discontinuous by nature, these ligament strips read as scattered bridges rather than a single continuous sheet. Clinically, the intertransverse ligaments matter because they define the intertransverse space that surgeons and proceduralists work around when approaching the posterior abdominal wall and lumbar spine, and they help segment the lateral boundary of the spinal motion segment during assessment of instability. Their relationship to the transverse processes makes them relevant landmarks in lateral lumbar exposures (for example, during transpsoas approaches) and in teaching where fascial planes and ligamentous restraints sit relative to the vertebral column. Small structure, real consequences. Sprain or hypertrophy here can contribute to localized paraspinal pain patterns and can be discussed alongside adjacent stabilizers such as the thoracolumbar fascia and quadratus lumborum.
