Description

Rising from the superior surface of the C2 vertebral body, the dens (odontoid process) projects cranially in the midline, with its anterior articular facet oriented toward the posterior surface of the anterior arch of the atlas (C1). Inferior to the dens, the axis body broadens, while the paired superior articular facets sit superolaterally, positioned to receive the inferior articular facets of the atlas. In an anterior perspective, the pedicles and transverse processes of C2 fall lateral to the vertebral body, framing the vertebral foramen posteriorly even when the posterior elements are largely out of plane. Coupling the skull base with the upper cervical spine in a low-angle composition makes the atlanto-axial complex easier to teach as a functional unit rather than as isolated bones. The dens is the key landmark: transverse ligament of the atlas failure or odontoid fracture can destabilize C1 to C2 rotation, and the anterior view helps clarify why Type II fractures at the base of the dens have a higher nonunion risk and drive management decisions. Small distances matter here. Misinterpret the midline and you misread stability.
