Description
Running along the lumbar spine, the vertebral bodies sit anteriorly as block-like segments separated by intervertebral discs, while the vertebral arches extend posteriorly to form the laminae and midline spinous processes. A focal region of fracture is emphasized in red on a single lumbar vertebra, centered on the posterior elements where the lamina meets the base of the spinous process. Superior and inferior articular processes frame the injured level, and the disc spaces above and below help localize the affected motion segment. Posterior element fractures matter because they change the stability story. Disruption of the lamina and spinous process can accompany flexion-distraction patterns (for example, seat-belt type injuries) or occur with burst fractures that propagate posteriorly into the canal, where retropulsed fragments and epidural hematoma become the concern. For surgical planning, the integrity of the posterior tension band guides whether you can manage with bracing or need posterior instrumentation, and it informs safe placement of pedicle screws relative to the lamina, pars interarticularis, and facet joints. Small details drive big decisions.
