Description
Lateral dorsal nucleus of the thalamus occupies the dorsolateral aspect of the thalamic mass in this superior perspective, appearing as an elongated nuclear territory running along the upper surface of the diencephalon. Medially, its boundary lies near the ependymal margin of the third ventricle and the medial thalamic surface, while laterally it approaches the thalamic interface with the internal capsule. Posteriorly it blends toward the pulvinar region, and anteriorly it abuts neighboring dorsal thalamic nuclei along the superior thalamic plane. Orientation is unambiguous. Clinically, the lateral dorsal nucleus is grouped with the limbic-associated thalamic nuclei, with reciprocal connections to cingulate and parahippocampal cortices that make it relevant when teaching circuits of memory, affect, and spatial orientation rather than primary sensory relay. A small paramedian or posterior thalamic infarct, deep venous thrombosis with thalamic edema, or a focal thalamic glioma can interrupt these networks and present with cognitive or behavioral change that is easily misattributed if learners only think in terms of VPL/VPM sensory syndromes. The superior surface emphasis helps separate dorsal thalamic nuclear territory from the lateral wall of the third ventricle and highlights how close this nucleus sits to major white matter outflow laterally, a practical consideration when correlating with axial MR images at the thalamic level.
