Description
Viewed from the lateral aspect of an adult male lower limb, the gluteus maximus and gluteus medius sit superiorly over the lateral hip, blending into the iliotibial tract as it descends along the lateral thigh to the anterolateral tibia at Gerdy’s tubercle. Anterior to the femur, the vastus lateralis and the iliotibial tract frame the lateral knee, while posteriorly the biceps femoris tendon approaches the fibular head, a key lateral landmark. Distal to the knee, the lateral compartment of the leg (crus) highlights fibularis (peroneus) longus and fibularis brevis hugging the fibula, with the lateral gastrocnemius and soleus forming the posterior calf and converging into the calcaneal (Achilles) tendon at the heel. Tendinous insertions are emphasized. Lateral anatomy matters because it is where common traction, compression, and overuse problems declare themselves. The peroneal (fibular) nerve’s relationship to the biceps femoris tendon and the fibular neck explains why fibular head trauma, tight casts, or prolonged kneeling can produce foot drop and sensory loss over the dorsum of the foot. This angle also clarifies the mechanical line of pull implicated in iliotibial band friction syndrome at the lateral femoral epicondyle and why peroneal tendinopathy and subluxation occur behind the lateral malleolus after inversion injuries.
