Description
Running obliquely from the upper thoracic and lower cervical region toward the skull, the longissimus capitis is presented in lateral profile as it ascends posterior to the cervical vertebrae and inserts on the mastoid process of the temporal bone, just posterior and inferior to the external acoustic meatus. Its belly sits deep to the trapezius and typically deep to the splenius capitis, while lying lateral to the semispinalis capitis and continuous inferiorly with longissimus cervicis along the posterior elements of the cervical spine. The cervical spinous processes and transverse processes provide bony reference points for the muscle’s superior course and for the layered arrangement of the posterior neck. Clear fiber direction matters. Clinically, this view helps when teaching posterior cervical muscle layering in patients with neck pain after flexion extension injury, where longissimus capitis strain can coexist with facet joint irritation and tenderness near the mastoid insertion. Because the muscle is supplied by dorsal rami of cervical spinal nerves, its pain referral and spasm patterns are often discussed alongside greater and lesser occipital nerve symptoms and cervicogenic headache workups. The mastoid process is also a practical palpation landmark when describing myofascial trigger points and planning targeted rehabilitation for extension and ipsilateral rotation deficits.
