Description

Seen from a posterior perspective in an adult male lower abdomen, the superior mesenteric vein and its tributaries course within the mesentery as they drain the jejunal and ileal loops, then ascend to the right of the superior mesenteric artery toward the portal venous confluence. Posteriorly, the inferior vena cava and abdominal aorta provide the central retroperitoneal landmarks, with mesenteric venous arcades and vasa recta radiating distally toward the intestinal wall. Portions of the colon frame the field laterally, while autonomic nerve plexuses track along the mesenteric vessels and mesenteric lymph nodes cluster near the root of the mesentery. Depth is clear. This posterior orientation is the one surgeons and anatomists reach for when teaching the relationship of the mesenteric vessels to the retroperitoneum and pancreatic head, where the superior mesenteric vein lies in the dissection plane for a pancreaticoduodenectomy and is vulnerable during uncinate process mobilization.
