Description
Arising between the right and left hepatic lobes, the middle hepatic vein courses superiorly and posteriorly within the main portal fissure toward its confluence with the inferior vena cava. From a posterior perspective, the vein lies medial to the right hepatic vein and lateral to the left hepatic vein, draining the central portions of the liver (segments IV, V, and VIII) before entering the hepatocaval region. The semi-transparent parenchyma allows the venous tributaries to be followed from deeper venules toward the larger collecting trunk. Orientation stays anchored by the posterior surface of the liver and the expected relationship of the hepatic venous outflow to the retroperitoneum. Surgical planning often hinges on this anatomy. During right or left hepatectomy and in split-liver graft procurement, the middle hepatic vein defines the plane between functional lobes, and inadvertent sacrifice can produce venous congestion of the anterior right lobe or segment IV depending on which tributaries are preserved. Short hepatic veins and the close proximity of the middle hepatic vein to the inferior vena cava also explain why bleeding in the hepatocaval junction can be difficult to control, and why preoperative mapping on contrast-enhanced CT or MR venography focuses on hepatic venous variants and accessory drainage pathways. This is a common pitfall. Use this posterior view in hepatobiliary surgery teaching files, anatomy courses covering liver segmentation (Couinaud), and medical publishing figures on hepatic venous drainage and venous outflow obstruction such as Budd-Chiari syndrome.
