Description
Arising from the central liver parenchyma, the middle hepatic vein (vena hepatica media) tracks superiorly and posteriorly toward the hepatocaval confluence, where it joins the inferior vena cava. From a superior perspective, the vein’s gentle arc is apparent as it courses between the functional right and left hemilivers, with smaller venules converging into it from adjacent segments. Translucent rendering of the hepatic substance allows the draining pattern to be read in situ rather than as an isolated vessel. Short hepatic veins can be inferred as additional tributaries entering the cava more laterally. For hepatobiliary surgery, this is the landmark plane. The middle hepatic vein approximates Cantlie’s line and often guides right versus left hepatectomy, while its segmental tributaries, commonly from segments V and VIII, are encountered during parenchymal transection and can drive major bleeding if misidentified. Venous outflow also frames transplant planning and postoperative complications: stenosis or thrombosis at the hepatic vein anastomosis produces hepatic venous outflow obstruction with congested segments that follow the vein’s drainage territory.
