Description

Seen from a superior perspective beneath the diaphragm, the left hepatic vein courses in the superior aspect of the left lobe and drains posteriorly into the suprahepatic segment of the inferior vena cava near the caval groove. Portions of the left and middle hepatic veins may appear approaching a common trunk, with smaller venules converging from the hepatic parenchyma. The left portal vein and its segmental branches are visible deeper and more anterior within the liver substance, contrasted against the hepatic venous outflow that lies closer to the diaphragmatic surface. Lobation cues help orient the viewer, with the left lobe positioned anterolaterally relative to the vena cava. A superior view is the one you want when teaching the outflow anatomy that matters during hepatic resection and transplant. The left hepatic vein defines venous drainage of segments II and III and sits close to the hepatocaval junction, where short hepatic veins and the inferior vena cava complicate mobilization of the liver from the diaphragm. Control of the left hepatic vein, and recognition of variants such as a shared trunk with the middle hepatic vein, directly affects bleeding risk during left hepatectomy, left lateral sectionectomy, or split-liver procurement. Bleeding here is unforgiving.
