Inferior Perspective of the Left Hepatic Vein

From an inferior perspective, the left hepatic vein is seen coursing superiorly and medially through the left lobe of the liver to join the inferior vena cava near the diaphragmatic surface. Tributary venules converge from the posterior and superior parenchyma, collecting into a larger-caliber vessel that lies anterior to the retrohepatic cava and deep to the visceral peritoneal covering. Its relationship to the falciform ligament and left sagittal fissure helps orient the segmental anatomy, with drainage patterns typically reflecting Couinaud segments II and III, and often part of segment IV. Surgeons and interventionalists care about this angle because the left hepatic vein is a key outflow landmark during left hepatectomy, living donor procurement of the left lateral segment, and venous reconstruction planning. Variants matter. An accessory left hepatic vein or a shared common trunk with the middle hepatic vein can change the safe line of parenchymal transection and influence the risk of hepatic congestion if outflow is compromised, an issue that becomes immediate on intraoperative Doppler ultrasound or venography.
