Description
Centered on the left lobe of the adult male liver, the left hepatic vein is traced from peripheral venules toward its broad collecting lumen as it courses superiorly and posteriorly to its termination at the inferior vena cava. Segmental tributaries converge within the parenchyma, with smaller veins running from lateral and inferior regions toward a more medial, cranial outflow channel. Semi-transparency of the hepatic tissue allows the relationship between intraparenchymal venous branches and the liver surface contours to remain readable. Blue vessel coding emphasizes venous drainage. Clear visualization of the left hepatic vein matters in hepatic resection planning and transplant anatomy because its confluence pattern and proximity to the left portal pedicle set the safe planes for left lateral sectionectomy and extended left hepatectomy. This is where surgical margins can become unforgiving. The vein’s short extrahepatic segment and wide ostium at the caval junction are also relevant during mobilization of the left lobe and when controlling bleeding near the hepatocaval interface, where injury can rapidly convert a controlled dissection into major venous hemorrhage.
