Description
Anterior orientation brings the left hepatic vein into clear relief as it courses superiorly within the left lobe, collecting smaller venules from the hepatic parenchyma before converging into a single, wider channel. Tributaries within segments II and III (and often the medial segment IV via a left or middle hepatic venous connection) run from inferior to superior and from lateral to medial toward the main trunk. Near the posterosuperior surface, the vein approaches the suprahepatic portion of the inferior vena cava at the hepatic venous confluence. Blue venous branches stand out against the translucent liver tissue. For teaching, the left hepatic vein is a practical landmark because it tracks the functional anatomy of the left liver and helps orient Couinaud segmentation along with the portal pedicles (even when the portal vein is not drawn). Surgeons planning a left lateral sectionectomy or left hepatectomy care about its caliber and tributary pattern, since injury at the hepatocaval junction can cause rapid, hard-to-control bleeding. Interventional radiologists also use the left hepatic vein as an access route for hepatic venous pressure gradient measurements and, in some patients, as a target during transjugular intrahepatic portosystemic shunt planning. Small variations matter.
