Running anterolateral to the cervical vertebral bodies, the middle part of the longus colli is presented in lateral profile against the stacked cervical spine. Its muscular belly lies deep to the prevertebral fascia, immediately anterior to the transverse processes and close to the anterior tubercles where the anterior scalene takes origin more laterally. Superiorly it approaches the mid-cervical levels beneath the skull base, while inferiorly it blends toward the lower cervical region at the thoracic inlet, where the first rib and upper thoracic cage begin to frame the neck. Bony context is prominent, including the mandible, cervical vertebrae, clavicle, scapula, and the upper ribs. Clinically, the longus colli matters because it defines the prevertebral plane used in the anterior cervical approach to the spine (Smith-Robinson), where dissection proceeds medial to the carotid sheath and lateral to the tracheoesophageal complex before the prevertebral fascia is opened. A small calcium hydroxyapatite deposition in its superior fibers can produce acute calcific tendinitis of the longus colli, a classic mimic of retropharyngeal abscess on lateral neck imaging with prevertebral soft-tissue swelling and odynophagia. Deep neck anatomy is unforgiving.
