Description
Running vertically along the anterior surfaces of the mid to lower cervical vertebral bodies, the middle straight (vertical) portion of longus colli lies deep to the prevertebral fascia and immediately anterior to the anterior longitudinal ligament. Superiorly it approaches the level of the hyoid region and skull base landmarks, while inferiorly it descends toward the cervicothoracic junction, medial to the clavicle and the root of the scapula. Lateral to this muscle belly, the prevertebral compartment blends toward the scalene region, while posteriorly the cervical vertebrae, including vertebral bodies and spinous processes, define the osseous corridor it stabilizes. Deep. Midline. Often overlooked. Anterior cervical spine exposure (Smith-Robinson approach) brings you directly onto longus colli, and the muscle’s medial borders are routinely elevated to place self-retaining retractors over the vertebral bodies and discs. That maneuver matters because the cervical sympathetic trunk runs on the prevertebral fascia along the lateral margin of longus colli, so excessive lateral dissection or retractor pressure can produce postoperative Horner syndrome. The vertical fibers also help explain why longus colli calcific tendinitis, typically near the superior oblique tendon at C1 to C2 but felt as deep anterior neck pain, can mimic retropharyngeal infection yet resolves with anti-inflammatory management rather than drainage. Use this illustration for teaching prevertebral layer anatomy in head and neck or spine modules, for operative atlases describing anterior cervical discectomy and fusion, or for radiology correlation when orienting learners to the prevertebral space on CT and MRI. It also fits well in patient-facing surgical consent materials where you need a clear depiction of what lies in front of the cervical vertebral column.
