Description
Anteriorly, the middle portion of the longus colli muscle is highlighted bilaterally along the anterior surface of the cervical vertebral column. Its vertical and oblique fibers lie deep to the prevertebral fascia, medial to the carotid sheath, and immediately anterior to the vertebral bodies and intervertebral discs, spanning the mid cervical levels where the muscle thickens between its superior and inferior attachments. Superiorly the skull base and anterior cervical spine frame the field, while inferiorly the view extends toward the thoracic inlet where the clavicles and first ribs begin to appear laterally. The midline stays bony and quiet. Emphasizing the middle longus colli in an anterior neck dissection context matters because this muscle forms the prevertebral bed encountered after mobilizing the pharynx and esophagus and retracting the carotid sheath laterally. For anterior cervical discectomy and fusion (ACDF), the longus colli is elevated from the vertebral bodies to place retractors, and the sympathetic trunk runs on or just lateral to its surface, a relationship that explains postoperative Horner syndrome when dissection strays too far laterally. This view also supports teaching prevertebral space infection pathways and the deep neck fascial planes that can channel retropharyngeal abscesses toward the mediastinum. Use it in gross anatomy lab manuals and head and neck modules to orient students to prevertebral muscles versus the more superficial infrahyoid and sternocleidomastoid layers, and in spine surgery texts to illustrate safe medial-to-lateral limits of longus colli elevation at the mid cervical levels.
