Description
Running in small paired slips between adjacent cervical transverse processes, the intertransversarii cervicis appear as short longitudinal columns flanking the vertebral column in the anterior neck. Medial to them sit the cervical vertebrae, with the vertebral bodies and anterior tubercles providing bony reference, while more superficial strap muscles and the sternocleidomastoid frame the region laterally. Superiorly, the mandible and oral aperture with teeth establish craniofacial orientation, and the hyoid bone hangs inferior to the floor of mouth, anterior to the upper cervical spine. Inferiorly, the anatomy blends into the thoracic inlet where the clavicles and first ribs border the lower neck. Fine as they are, the intertransversarii matter because they sit at the interface of segmental motion and stability in the cervical spine, acting with the multifidus and rotatores to control coupled lateral flexion and rotation across individual motion segments. This anterior perspective is useful when teaching why deep paraspinal muscle dysfunction can follow whiplash injury, and why persistent neck pain often correlates with impaired cervical motor control rather than gross weakness of the superficial muscles. Their relationship to the transverse processes and intertransverse spaces also helps explain landmarks used when planning cervical facet interventions and when navigating tissue planes for an anterior cervical approach, where deeper stabilizers lie posterior to the prevertebral fascia and longus musculature.
