Description
Deep to the superficial cervical layers, the intertransversarii cervicis are rendered as short segmental muscle slips spanning between adjacent transverse processes of the mid to lower cervical vertebrae, lying lateral to the vertebral bodies and intervertebral discs. Superiorly, the chain approaches the atlantoaxial region beneath the cranial base, while inferiorly it descends toward the cervicothoracic junction above the first ribs. Nearby landmarks include the maxilla and dentition anteriorly, the nasal cavity superiorly, and the laryngeal framework (thyroid and cricoid cartilages) positioned anterior to the cervical spine, with the clavicles and upper rib articulations marking the inferior boundary of the field. Segmental neck stabilizers matter most when you are trying to explain motion between individual vertebrae rather than gross flexion or rotation. Intertransversarii coordinate with the deep paraspinals and prevertebral muscles to control side-bending and limit unwanted shear across uncovertebral and facet joints, a teaching point that becomes concrete in whiplash-associated disorders and chronic mechanical neck pain. Their close relationship to the transverse processes also makes them a relevant reference in procedures that use those processes as landmarks, including cervical medial branch blocks and posterior approaches where accurate depth orientation helps avoid drifting toward the exiting cervical nerve roots.
