Description
Along the lateral aspect of the cervical spine, the intertransversarii cervicis appear as small segmental muscle slips spanning adjacent transverse processes from the upper to lower cervical vertebrae (C2 to C7), positioned deep to the more superficial posterior neck muscles and close to the intertransverse spaces. Superiorly, the skull base and atlanto-occipital region frame the top of the field, while inferiorly the cervicothoracic junction blends toward the first ribs and the shoulder girdle. Each bundle sits immediately lateral to the articular pillars and facet joints, with the transverse foramina and their vascular contents lying anteromedial to the muscle at levels C1 to C6. Intervertebral discs and vertebral bodies provide the midline reference, while the red paraspinal musculature emphasizes the deep stabilizing layer. Because the intertransversarii are short, proprioceptive muscles rather than prime movers, this view is most useful when teaching segmental control of the cervical spine and how small asymmetries can influence coupled motion (lateral flexion with rotation) during head turning. Their relationship to the zygapophysial (facet) joints matters in clinical neck pain syndromes, where facet arthropathy, whiplash-associated disorders, or muscular guarding can all localize tenderness just lateral to the spinous processes.
