Description
Spanning the posterior torso of an adult male, the superficial and intermediate back muscles are arranged around the midline spinous processes of the cervical, thoracic, and lumbar vertebrae. Superiorly, trapezius fans laterally from the nuchal region and thoracic midline toward the acromion and spine of the scapula, with rhomboid major and minor lying deep to it and running inferolaterally from the medial border of the scapula to the upper thoracic vertebrae. Inferior and lateral to the scapulae, latissimus dorsi sweeps superolaterally from the thoracolumbar fascia and iliac crest region toward the proximal humerus. Along either side of the vertebral column, the erector spinae mass forms paired longitudinal columns that frame the midline. Clear surface anatomy. Palpation and intervention depend on these relationships: the medial scapular border overlies the rhomboids, while trapezius covers the region where the spinal accessory nerve travels on the deep surface before entering the muscle, a point of vulnerability in posterior cervical triangle surgery and during lymph node biopsy. The lateral free edge of latissimus dorsi and the thoracolumbar fascia define planes used in posterior trunk approaches and in harvesting a latissimus flap for breast or chest wall reconstruction. For teaching biomechanics, the juxtaposition of erector spinae to the spinous processes helps explain extension moments and why paraspinal strain commonly localizes just lateral to the midline in lumbar overuse and poor lifting mechanics. Use this artwork in gross anatomy and kinesiology modules to correlate surface landmarks with scapulothoracic motion, and in radiology or rehabilitation publications that introduce muscular contributors to scapular dyskinesis and thoracolumbar pain patterns.
