Description
Running over the dorsum and plantar surface of the adult male foot, the vascular map centers on the dorsal venous arch superficial to the metatarsals and the plantar venous plexus deep to the plantar aponeurosis, with perforating veins linking the two across the intermetatarsal spaces. Anteriorly at the ankle, the anterior tibial artery continues as the dorsalis pedis artery between the tendons of extensor hallucis longus and extensor digitorum longus, giving arcuate and dorsal metatarsal branches that course distally toward the proximal phalanges. Plantar to the tarsus, the posterior tibial artery divides into the medial and lateral plantar arteries, the latter sweeping laterally and then medially to form the plantar arterial arch alongside the deep plantar branch of the dorsalis pedis. Superficial venous channels track toward the medial marginal vein (continuing proximally as the great saphenous vein) and the lateral marginal vein (to the small saphenous vein). Clear surface anatomy. That relationship between dorsalis pedis and the plantar arch is the key teaching point for collateral flow, and it matters every time a clinician palpates pedal pulses or plans an incision across the midfoot. Diminished dorsalis pedis pulse in peripheral arterial disease, diabetic macrovascular disease, or acute limb ischemia is often assessed against posterior tibial and plantar perfusion, while superficial venous pathways explain common sites of varicosities and the routes of cellulitis spread along the dorsal foot. Surgeons also use these landmarks to avoid vascular injury during dorsal approaches to the talonavicular and tarsometatarsal (Lisfranc) joints.
