Deltoid Fascia in a Full Body View

Spanning the lateral shoulder as a thick connective tissue membrane, the deltoid fascia forms a dense sleeve over the deltoid muscle and continues superiorly to blend with the pectoral fascia anteriorly and the fascia overlying trapezius posteriorly. From a full body male view, you would expect the fascia to be emphasized over the shoulder cap, with the clavicle and acromion lying superior to it and the proximal humerus deep and inferior at the level of the deltoid tuberosity. Laterally, the fascial sheet drapes over the contour of the deltoid belly; medially it tightens toward its attachments along the scapular spine, acromion, and lateral clavicle. Deep to this layer sit the subdeltoid (subacromial) bursa and rotator cuff tendons, separated from the skin by superficial fascia and the deltopectoral region anteriorly. Understanding the deltoid fascia matters because it defines surgical and injection planes around the glenohumeral joint and influences how fluid, hemorrhage, or infection tracks in the shoulder. The deltopectoral approach exploits the interval between deltoid and pectoralis major where the cephalic vein runs in superficial fascia, while transdeltoid approaches must respect the course of the axillary nerve and posterior circumflex humeral vessels on the deep surface of deltoid. A tight fascial envelope also helps explain why expanding lesions in the deltoid compartment can be painful.
