Running along the anterior compartment of the male leg, the extensor hallucis longus arises from the middle third of the fibular shaft and adjacent interosseous membrane, then narrows into a tendon that tracks inferiorly toward the dorsum of the foot and the great toe. Medial to it, the tibia forms the dominant shin contour, while the fibula remains more lateral and slender, creating the bony corridor where the muscle belly typically sits. Distally, the tendon passes anterior to the ankle joint, crosses the talar dome region, and continues to the dorsal aspect of the hallux, with the tarsals, metatarsals, and phalanges providing clear osseous landmarks for orientation. An anterior perspective makes the tendon’s course and its relationship to the tibia and fibula easy to teach. Clean landmarks. Clinically, the extensor hallucis longus is a key examiner’s muscle because its tendon becomes a surface guide for the deep fibular (peroneal) nerve and anterior tibial artery as they traverse the anterior ankle and dorsum of the foot. The tendon’s passage beneath the extensor retinacula also matters in anterior ankle impingement and in surgical approaches to the ankle and first ray, where respecting the tendon and neurovascular bundle avoids iatrogenic injury.
