Description
Projected on a semi-transparent thoracic cage and vertebral column, the right lobe of the liver appears in the right upper quadrant with the gallbladder (vesica biliaris) tucked on its visceral (inferior) surface in the gallbladder fossa. A lateral profile makes the fundus read as the most anterior and inferior portion, while the body and neck taper posteriorly and medially toward the cystic duct. From there, the cystic duct joins the common hepatic duct to form the common bile duct, descending toward the second part of the duodenum behind the costal margin. A side, x-ray style presentation suits the common clinical question of where gallbladder pain lives and where the ducts run when you cannot see them directly. Biliary colic and acute cholecystitis often localize to the right upper quadrant with referral to the right shoulder via diaphragmatic irritation, and the cystic duct and common bile duct are the channels that matter when stones migrate. Orientation also supports operative thinking: during laparoscopic cholecystectomy, the surgeon works around the neck of the gallbladder and hepatocystic triangle, where misidentification of the common bile duct remains a feared complication.
