Description
Posterior hip and buttock anatomy is centered on the gluteus maximus, the thick superficial extensor of the hip, with its broad origin spanning the posterior ilium, dorsal sacrum, and coccyx and converging inferolaterally toward the iliotibial tract and gluteal tuberosity of the femur. Laterally, the iliotibial tract continues distally along the thigh as the fibrous reinforcement of the fascia lata, while the inferior border of the muscle forms the gluteal fold overlying the proximal posterior thigh. Medially, the sacrum and coccyx anchor the muscle’s aponeurotic fibers, and superiorly the posterior iliac crest defines the upper limit of the gluteal region. Powerful architecture. Gluteus maximus morphology matters because its dual insertion determines force transmission for hip extension and pelvic control, a distinction that comes up in both gait analysis and operative planning. Tightness or overactivity can contribute to iliotibial band friction symptoms laterally at the knee, while weakness is a common driver of posterior chain compensation, anterior pelvic tilt, and difficulty rising from a chair or climbing stairs. For surgeons, the muscle’s bulk and fiber direction frame familiar corridors for posterior hip approaches and influence where retraction risks injury to the inferior gluteal nerve and adjacent sciatic nerve.
