Iliacus Beneath the Skin

Description

Displayed beneath the intact integument, the iliacus occupies the iliac fossa on the internal surface of the ilium, spreading broadly from the iliac crest and ala and tapering inferiorly toward the musculotendinous junction with psoas major. Lateral to psoas major, its fibers converge and pass deep to the inguinal region to join the iliopsoas tendon, continuing to the lesser trochanter of the femur. Posteriorly, the muscle relates to the iliac bone and anterior sacroiliac region, while anteriorly it lies deep to the abdominal wall and iliac fascia. Understanding the iliacus in a lateral hip and lower back context helps clarify why the iliopsoas behaves as more than a simple hip flexor. Tightness or spasm can increase anterior pelvic tilt and lumbar lordosis, a common mechanical pattern in groin pain and low back pain presentations, and hypertrophy or hematoma within the iliopsoas compartment can irritate the femoral nerve as it courses between iliacus and psoas. This is also the corridor used in anterior approaches to the hip and in retroperitoneal exposure of the iliac vessels, where staying oriented to the iliacus fascia and the pelvic brim matters.
