Description
Seen from a posterior perspective through partially transparent skin, the upper thoracic back is opened just enough to expose the levatores costarum as paired slips running inferolaterally from the transverse processes to the ribs. Each muscle arises from the transverse process of T1 to T11 and inserts onto the rib below, near the angle, lying deep to the erector spinae mass and just lateral to the thoracic spinous processes. The thoracic vertebrae, ribs, and costovertebral joints frame these fibers, with the erector spinae columns positioned bilaterally and more superficial. Orientation matters. The levatores costarum often get conflated with the intercostals or with posterior serratus muscles, yet their transverse process to rib attachment pattern makes them a clean teaching example of how the thoracic spine couples segmental motion with rib excursion. Their small size does not make them trivial. In thoracic paraspinal pain syndromes and in segmental dysfunction around the costotransverse joints, these slips can be implicated as local tension generators, and they form part of the deep muscular layer a surgeon or proceduralist passes when working near the posterior thoracic elements.
