Description
Viewed from a superior (cranial) perspective, the trachea descends in the midline to the carina, where it bifurcates into the right and left main bronchi before entering the pulmonary hila. Each lung is rendered with lobar contour and fissure lines, with bronchial branches extending distally into the lung parenchyma toward the alveolar region. A malignant mass occupies a focal portion of the pulmonary tissue, positioned in close apposition to a segmental bronchus, making its relationship to the conducting airways clear in three dimensions. Surface vessels track over the visceral pleura, reinforcing the anatomic orientation of the lobes. Top-down anatomy matters for lung cancer because many bronchogenic carcinomas arise from the bronchial epithelium and extend along bronchi or obstruct them, producing post-obstructive atelectasis, recurrent pneumonia, or hemoptysis. Proximity of the tumor to a lobar or segmental bronchus also frames procedure planning, whether the question is bronchoscopic biopsy and stent placement, a sleeve resection to preserve parenchyma, or the bronchial margin required for lobectomy.
