Description
Arising within the infratemporal fossa, the maxillary vein is shown as a blue-caliber vessel emerging from the pterygoid venous plexus that lies medial to the ramus of the mandible and adjacent to the lateral pterygoid muscle. From this plexiform bed, tributaries converge posteriorly toward the neck of the mandible, where the vein courses toward the parotid region to contribute to formation of the retromandibular vein. Superficial and deep relationships are legible against a lateral skull profile, with venous channels also traced around the orbit and nasal skeleton for orientation. Teeth, maxilla, and mandible provide fixed bony landmarks. Anatomists and surgeons care about this segment because the pterygoid plexus and maxillary vein sit at the crossroads between deep facial drainage and intracranial venous pathways. Small emissary connections to the cavernous sinus, paired with continuity to the facial and ophthalmic venous systems, explain how odontogenic infection or facial trauma can propagate beyond the masticator space. Bleeding can be brisk. The same anatomy matters during posterior maxillary dental local anesthesia (pterygoid plexus hematoma), open reduction of mandibular condylar fractures, and endoscopic or open approaches to the infratemporal fossa.
