Posterior anatomy of an adult male upper trunk is presented with the trapezius dominating the superficial layer, spanning from the external occipital protuberance and nuchal ligament inferiorly to the acromion and spine of the scapula laterally. Inferior to it, the upper fibers of the latissimus dorsi and the thoracolumbar fascia frame the paraspinal groove where the erector spinae group lies deep and medial to the scapular stabilizers. Along the scapula, infraspinatus occupies the infraspinous fossa while teres minor sits superior and teres major inferior near the lateral border, converging toward the proximal humerus beneath the posterior deltoid. Parts of the sternocleidomastoid and posterior cervical musculature contour the neck superiorly. Clear bony landmarks. This posterior shoulder girdle view matters because it ties surface anatomy to the osseous attachment sites that guide exam and intervention, from palpating the scapular spine and medial border to understanding how the trapezius and serratus anterior couple for upward rotation during arm elevation. Rotator cuff pain is often localized by relating infraspinatus and teres minor to the greater tubercle, while strain patterns in the upper trapezius and levator scapulae commonly accompany cervical radiculopathy or scapular dyskinesis. The relationship of paraspinal muscle columns to the spinous processes also supports teaching of midline approaches and safe zones for trigger point injection.
