Description
Pia mater closely invests the cerebral hemispheres, draping over the gyri and descending into the sulci to follow the cortical surface without bridging across fissures. Along the convexity, pial arteries and veins arborize within the subarachnoid space, their branches tracking the sulcal margins before penetrating perpendicularly as cortical perforators. Inferiorly, the membrane continues onto the brainstem, where cranial nerves emerge from the ventral pons and medulla and course anterolaterally from their root entry and exit zones. Color coding separates arterial, venous, and neural elements for rapid orientation. Teaching the pia is rarely about a standalone layer, it is about the interface where neurosurgeons, neurologists, and neuroradiologists anchor their thinking about subarachnoid hemorrhage, leptomeningitis, and pial collateral flow. Blood in a subarachnoid hemorrhage tracks the sulci and basal cisterns because it lies deep to the arachnoid and superficial to the pia, and this view makes that spatial logic easy to explain. The close relationship between pial vessels and the cortical surface also frames why vasospasm after aneurysmal rupture can produce delayed ischemia, and why pial arterioles matter in superficial cortical infarcts. Neuroanatomy and neuropathology courses use this rendering to contrast dura, arachnoid, and pia, and to map where CSF, blood, and inflammatory exudate accumulate in common intracranial emergencies. It also fits neurosurgical teaching on sulcal dissection planes, cortical vessel preservation during craniotomy, and correlation with CTA or MRA surface angiography of the cerebral convexity and brainstem.
