Description
Posteriorly, the pia mater invests the cerebrum and cerebellum as a thin leptomeningeal layer that conforms tightly to gyri and sulci, continuing into the deeper fissures rather than bridging across them like the dura. Superiorly, it follows the medial margins of the cerebral hemispheres toward the interhemispheric fissure; inferiorly, it drapes over the cerebellar hemispheres and vermis and continues caudally toward the cervicomedullary junction and upper spinal cord. Pial vessels course within this membrane, with cortical arterial branches and superficial cerebral veins running on the brain surface before converging toward dural venous sinus drainage pathways. Color coding separates arteries, veins, and cranial nerve elements. Clear. Understanding the pia from a posterior view matters because it is the layer that directly accompanies penetrating vessels into the parenchyma, a relationship central to subarachnoid hemorrhage patterns, vasospasm risk after aneurysmal rupture, and the spread of leptomeningeal infection or carcinomatosis along cisternal spaces.
