Description
Across the anterior abdominal wall, the paired rectus abdominis muscles run vertically from the pubic crest and pubic symphysis inferiorly toward the costal cartilages of ribs 5 to 7 and the xiphoid process superiorly. Medially, each muscle belly approaches the linea alba, while its lateral margin defines the rectus sheath formed by the aponeuroses of the external oblique, internal oblique, and transversus abdominis. Transverse tendinous intersections segment the muscle into stacked bellies, creating the familiar “six pack” surface relief. Clear landmarks. This subcutaneous emphasis on fiber direction and tendinous intersections matters when teaching why the rectus abdominis does not behave as a single uniform strap and why surgical incisions are planned around midline and paramedian planes. Midline laparotomy exploits the relatively avascular linea alba, while paramedian approaches and rectus-splitting techniques must respect the segmental neurovascular supply entering the rectus sheath laterally, as well as the superior and inferior epigastric vessels that course deep to the muscle. Diastasis recti, common after pregnancy but also seen in men with central obesity, specifically widens the linea alba without tearing the rectus bellies, and this view makes that distinction easy to explain.
