Description
Spanning the anterior compartment of the thigh, the vastus intermedius lies deep to rectus femoris and directly against the anterior and lateral surfaces of the femoral shaft. Proximally, its muscle fibers arise broadly from the proximal two thirds of the femur and the lateral intermuscular septum, then converge distally into the quadriceps tendon. Distal to the femoral condyles, its tendon fibers contribute to the patellar base and continue via the patellar ligament to the tibial tuberosity. Depth relationships matter here. Clinically, separating rectus femoris from vastus intermedius clarifies why an isolated rectus femoris strain can spare knee extension strength, while quadriceps tendon disease and extensor mechanism ruptures often implicate the deeper vastus intermedius contribution even when superficial contours look intact. An anterior thigh approach to the femur (for fracture fixation or intramedullary nailing) traverses the quadriceps mass, and appreciating the muscle’s broad femoral attachment helps anticipate bleeding planes and postoperative weakness.
