Description
Palmar structures dominate this anterior aspect of the male hand and wrist, with the flexor retinaculum spanning the carpal arch and the palmar aponeurosis fanning distally toward the digits. Distal to the carpus, the thenar eminence sits radial and the hypothenar eminence ulnar, framing the central flexor tendons as they course superficial to the metacarpals and into the fibrous digital sheaths. Along the fingers, the flexor digitorum superficialis splits to allow the flexor digitorum profundus to pass to the distal phalanges, while the metacarpophalangeal and interphalangeal joint capsules are reinforced by palmar plates and collateral ligaments. Short intrinsic muscles, including the lumbricals and interossei, relate closely to the metacarpals and digital rays, shaping the palmar contours. Palmar anatomy is where hand function and hand pathology meet. The relationship of the median nerve and long flexor tendons within the carpal tunnel is a common teaching point when explaining carpal tunnel syndrome, tendon synovitis, and postoperative scarring after carpal tunnel release. Fibrous continuity from the palmar aponeurosis into the digital fascia also underpins the contractures of Dupuytren disease and the way those cords tether the ring and small fingers into flexion.
