Opponens Digiti Minimi, Anterior View

Description

Arising from the hook of the hamate and the flexor retinaculum on the ulnar side of the carpus, the opponens digiti minimi is highlighted on the palmar (anterior) aspect of a male hand, its fibers coursing distally and laterally to run roughly parallel with the shaft of the fifth metacarpal. The muscle lies deep to the palmar fascia and hypothenar fat and sits radial to the abductor digiti minimi, while remaining superficial to the fifth metacarpal and the adjacent carpometacarpal joint. Medially, its origin neighbors the structures of Guyon’s canal, with the ulnar nerve and ulnar artery passing just proximal to the hypothenar mass before the deep branch of the ulnar nerve dives toward the interossei and adductor pollicis. Opposition of the little finger is a subtle movement, but it is the movement that lets the ulnar border of the hand rotate and “cup” against the thumb during power grip. Loss of opponens digiti minimi bulk and strength is a common teaching clue for deep ulnar neuropathy at the wrist (Guyon’s canal syndrome) and can help localize lesions compared with more proximal ulnar nerve injury at the cubital tunnel. This view also supports surgical planning: decompression at Guyon’s canal and approaches around the hook of hamate demand respect for the deep motor branch as it curves around the hamate into the palm.
