Description
Arising from the hook of the hamate and flexor retinaculum on the ulnar side of the palm, the opponens digiti minimi is presented in anterior (palmar) aspect as a thin, triangular hypothenar muscle running distally and laterally toward the fifth metacarpal. Superficial to it lies the palmar aponeurosis and, more ulnarly, the palmaris brevis, while flexor digiti minimi brevis and abductor digiti minimi sit adjacent within the hypothenar eminence. The tendon of flexor digitorum profundus to the little finger passes distally toward the digit, and the ulnar neurovascular bundle courses superficial and medial in Guyon canal before dividing into superficial and deep branches near the hypothenar muscles. Opposition of the small finger depends on the opponens digiti minimi rotating and flexing the fifth metacarpal at the carpometacarpal joint, bringing the digit across the palm to meet the thumb for power grip. Weakness in this motion helps localize ulnar nerve lesions distal to the elbow, and this palmar view clarifies why compression in Guyon canal can spare forearm flexors yet disrupt fine intrinsic hand function. A tight surgical field. The relationship of the muscle belly to the hook of the hamate is also a practical landmark during hypothenar flap elevation and in approaches to hook of hamate fracture. Hand anatomy instructors can pair this plate with thenar musculature diagrams to teach opposition as a coupled thumb and little finger action in gross anatomy and kinesiology courses.
