Beneath the palmar skin of the ulnar hand, the hypothenar eminence is opened to expose the opponens digiti minimi as a deep, triangular muscle running along the ulnar border of the fifth metacarpal. Superficial to it, the abductor digiti minimi and flexor digiti minimi brevis occupy the medial (ulnar) margin of the palm, while the flexor tendons and thenar mass lie more lateral toward the thumb. Proximally, the muscle fibers arise near the hook of hamate and adjacent flexor retinaculum, then sweep distally to insert along the medial surface of metacarpal V at the carpometacarpal region. Small bones and joint lines of the carpus, metacarpals, and phalanges provide clear spatial landmarks. Deep in the compartment, the deep branch of the ulnar nerve and ulnar artery course toward the interossei. A compact corner of the palm.
