Description
Arising from the flexor retinaculum and the hook of the hamate, the opponens digiti minimi runs distally along the ulnar (medial) border of the palm to insert on the ulnar side of the fifth metacarpal, lying deep to the abductor digiti minimi and superficial to the flexor brevis digiti minimi. From a lateral perspective, its fusiform belly sits anterior to the hypothenar metacarpal shaft and is framed proximally by the pisiform-hamate region, with its fibers oriented obliquely to pull the fifth metacarpal into flexion and rotation for opposition. The ulnar neurovascular bundle courses nearby in Guyon canal. Small space, tight anatomy. This angle matters when you are teaching why the little finger can cup toward the thumb, because the opponens acts on the fifth metacarpal at the carpometacarpal joint rather than directly flexing the phalanges. Ulnar nerve lesions at the wrist (Guyon canal compression, handlebar palsy) denervate hypothenar muscles, and loss of opponens digiti minimi weakens key pinch and power grip by flattening the ulnar palm. Surgeons also use these relationships during ulnar-sided wrist approaches and hypothenar muscle flaps, where preserving the deep branch of the ulnar nerve is the operative constraint. Use this asset in upper-limb anatomy and hand biomechanics teaching, in operative technique chapters on Guyon canal decompression, or in rehabilitation materials that distinguish intrinsic muscle deficits from flexor tendon dysfunction.
